Discussion: December Meeting Summary/Notes
“What is care coordination?” and “How care coordination services are delivered.”
Definitions:
- Care Coordination Services are person centered focused on the identification and elimination of barriers to optimized health outcomes to achieve improved quality of life for Oklahomans with Dual eligibility.
- Care Coordination Services are delivered by a team that streamlines clinical, program, benefits and community support and services. They are knowledgeable experts that can effectively improve quality Care through benefits coordination at a reduced cost.
